Sample Contract
The following is a schedule of (name
deleted) journey to freedom:
November 1, 2008 – November 1, 2009
1.
….. will turn over all cash and checks to Project Rescue.
These funds will be deposited into his account.
These funds will be turned over to ….. on
November 1, 2009, if he decides to move out of our apartments and
when he has paid off all his expenses.
If ….. exits the Project Rescue Program for any reason before
November 1, 2009, these funds will be turned over to his parents on
November 1, 2009.

From November 28, 2008 thru February 28, 2009 the A to B rule
will be in effect (calling Chris or Ronnie when traveling from one
place to another).
3.
On February 28, 2009 ….. will be given a debit card with a
maintained balance of $100. For the
duration of this contract, ….. will have access to his saved funds
for necessities (with Ronnie approval).
4.
On February 28, 2009 the A to B
rule will end. ….. will be able to go anywhere with anyone at any
time. He still must attend all Project
Rescue meetings and attend all church services on Wednesdays and
Sundays.
5.
On December 1st, ….. himself will begin to be
responsible for the Project Rescue $500 monthly fee.
The $500 monthly fee is not considered as a payment of rent.
It is the program fee. ….. will
pay for his own food, gas, and all other wants and needs.
6.
As of February 28, 2009 ….. can relocate anywhere he chooses.
All other provisions of this contract will be in effect until
November 1, 2009. If and when …..
relocates, his monthly program fee will be reduced to $300 a month
for the duration of this contract. …..
will be responsible for all his relocation expenses.
If and when ….. relocates, he will still be required to
attend Sunday & Wednesday church services.
Via webcam, ….. will attend the Tuesday, Friday and Saturday
evening Project Rescue meetings.
7.
….. himself and the other residents of the Jacksonville
mobile home will split the utility expenses, cable, internet
service, and garbage expenses.
I, …..
agree to
every part of this plan and schedule. I
will keep the rules of the Project Rescue program.
I realize that if I relapse on alcohol or drugs and/or fail
to perform all the requirements of the Project Rescue program, I
will lose all privileges, have to quit my job, start the program
over, and have to pay (out of my savings) the $500 monthly fee for
my stay in the program.
